Nomination Forms

The purpose and aim of the Johnson C. Smith University Athletics Hall of Fame is to recognize and honor those persons whose outstanding contributions have enriched the athletics program of Johnson C. Smith University and to provide an enduring memorial and preserve for posterity the memories of those persons whose endeavors brought fame and positive recognition to the University.
 
Each year, the JCSU HOF Committee will select a minimum of seven individuals (one from each of three categories) and a maximum of 10 individuals (excluding teams) to be inducted into the Johnson C. Smith University Athletics Hall of Fame. Only one team may be inducted into the Hall of Fame each year.

Guidelines for Nominations

Former Student-Athletes - Must be a JCSU graduate (may be waived by HOF committee in special cases). There must be a ten-year time lapse since candidate last participated in athletics at JCSU. Athletic achievements are of prime importance, but accomplishments after leaving JCSU will also be considered. Former athletes serving on the HOF committee may not be considered for induction.
    1. Must have achieved one or more of the following:
        a. All-American
        b. Participated in the Olympics or an Olympian
        c. All-Conference
        d. NCAA regional or national recognition in their respective sport

Coaches - Consideration for selection includes recognition as a leader, length and quality of the coaching period, program growth, and achievement of the individual after leaving JCSU.
    1.  The coach is recognized as a leader in the sport involved and has demonstrated length and quality of developing a program, achievement in their coaching record and service at the institution;
    2. Recognized as CIAA Conference, regional or national Coach of the Year or has received national organization acknowledgment;     
    3. Coach of a CIAA Championship Team(s) or National Championship team.

Team Nominations - If Nominating a team, please nominate the team NOT each individual on the team. (In a separate sheet list the team members). 
    1. Must have won a championship in the CIAA, NCAA Regional or National Championships.
            Other Points of Interest
            1. Teams shall receive one award which represents all participants and coaches of the respective team.  One team representative shall serve as spokesperson during the Hall of Fame event.

Others - Other non student-athletes/coaches may be considered based on exceptional contributions to the athletic program.

Posthumous - Deceased candidates shall be given equal consideration (waiting period may be waived).

Completing Nomination Form: Form must be completed with accuracy, neatness and all requested necessary information. Nomination forms that are not completed according to the instructions provided will not be considered.
